Eligibility requirements: Must have diagnosis of cancer. A
physician's prescription may be required. Must live, work, or be treated in
service area.
Intake Procedure: Call or walk in; a client information form
must be filled out.
Area Served: Delaware, Henry, & Randolph Counties
About the program: This program is designed to lend equipment
for the home in order to promote comfort and facilitate care. Items are lent upon
approval or recommendation of attending physician or medical personnel. Hospital
beds, walkers, bedpans, elevated tiolet seats, commodes, shower seats, disposable
items, and pads may be available. Other items include wigs, turbans, and
nutritional supplements.
Clients who have a prescription from a physician and who meet financial
guidelines may be referred to the Comrehensive Breast Health Center at Ball Memorial
Hospital for a diagnostic or screening mammogram.
The Client Assistance Program provides monetary assistance for medications to
manage cancer-related symptom and/or pre-cancer determined procedures based on
financial eligibility and availability of funds. Gas vouchers are available for
qualified clients who are receiving cancer treatments and using their own vehicles.
